Your Care Plan

Having a Care Plan is the best way of looking after your medical conditions. It is your personal plan for a healthier life.

The Investigator Clinic runs dedicated clinics with our experienced Care Plan Nurses working in consultation with your regular General Practitioner to create your personal plan.

Our Care Plan appointments are available to everyone who meets the medicare criteria and are bulk-billed as standard. They also enable you to access a Medicare rebate to see a Private Allied Health Practitioner for 5 visits per year.

What’s covered?

Conditions that require ongoing care, monitoring or management are eligible for a care plan. This includes everyone with diabetes or who has ever had a heart attack or a stroke. Please see below for a list of other common conditions included.

We know from the substantial scientific evidence that patients with conditions that cause long term effects have improved outcomes and a healthier life when they have an ongoing Care Plan program. Please discuss with your doctor who will be happy to confirm your eligibility and help you get started.

  • Palliative (end of life) care
